Community Care Direct Services provision - you will be expected to work across the borough as and when required by the needs of the people we support.

The purpose of this role is to lead & manage the delivery of high quality services for vulnerable adults within the borough that aim to improve their independence and quality of life experiences.

Community Independence Service

• To facilitate group and individual activities that support positive contributions to local communities e.g. volunteering. • To support individuals to travel independently various methods of independent travel including: the use of public transport, cycling or walking. • To deliver task focussed activities that enable people to develop the skills they require to live as independently as possible such as looking after my home, shopping and meal preparation and managing money etc. • To wherever possible to follow structured competency based training and educational opportunities that the service provides. • To ensure that each individual within the service has a specific outcome/s identified that they wish to achieve, in consultation with a named social worker and to monitor their progress • To encourage naturally occurring relationships, support and resources, so that an individuals network within the community are maximised • To prepare service users with necessary life skills to move on to more independent accommodation and to support them with this process.

Hours:37 hours per week; 5 days across 7 days a week between the hours of 7.30 a.m. and 7.30 p.m. (there may be a requirement to occasionally work outside the core hours)
