Consultant Community Social Worker - Afan Network

Salary: £37,890 - £41,881 per annum

The Afan Network team are looking for a dynamic and enthusiastic Consultant Social Worker to join a busy generic social work team covering the Port Talbot area. The vacancy has become available to join a committed and motivated management team who work hard to deliver excellent standards in a positive working environment.

The Afan Network Team works with adults with non-complex needs over the age of 18, who live at home across the borough of Neath Port Talbot. This includes people living with Dementia related conditions.

The Network Team works in partnership with individuals to identify what matters to them, devise care and support plans to meet their outcomes whilst maximising independence where possible. The focus of the team is to build on the assets of individuals within their own communities, and working with colleagues to identify this where appropriate.

The post holder will be expected to supervise and support team members in carrying out tasks, to report to the team manager and senior management regarding the progress of the team, and to manage more complex cases and investigate complaints. The post holder will also promote training and development on an individual basis as well as on a team level.

We work alongside our colleagues in Children’s Services to support young persons aged 16-18 years with non-complex needs who are in transition to adulthood, in order to meet their care and support needs.

The post holder will need to have in-depth knowledge and working practice of the Social Services and Well-being (Wales) Act 2014 to include outcome focussed assessment and care planning, knowledge and working practice of Continuing Health Care, Mental Capacity Act 2005 and Deprivation of Liberty Safeguards, and have knowledge and an understanding the relevance of Multi-Disciplinary working
