The Community Wellbeing Team is a multidisciplinary, integrated service that works closely with the Swansea Bay University Health Board. The team provides a Reablement service as well as short and long term care to residents within Neath Port Talbot.

With the on-going crisis of the COVID-19 pandemic, the Community Wellbeing Team is a business critical service, providing continued essential care and reablement to vulnerable residents of NPT, who still vitally need this service. Therefore, we would welcome applications from those who may have previously left the service and are interested in returning, or have prior experience of working in a similar setting. The usual working pattern is outlined above, but the service is able to be flexible with the hours of work.

All applicants need to be able to:

• work as an effective team member of a multi-disciplinary, multi-agency team in co-ordinating the above

• work as a member of the team helping to deliver a short/long term programme of care/therapy to clients who have become unwell or lost their skills and confidence for living independently.

• provide a high standard of personal care and support to people within their own home.

• take direction from supervisors/therapists to implement planned programmes of therapy/care from support plans; and

• Ensure the delivery of a high quality service in line with Registration and Inspections Social Care (Wales) Act 2016, statutory duties, policies, and procedures.

To undertake this challenging role, you must have experience of working with individuals in need of care or support in a health and social care setting, must have an understanding of supporting people living with dementia. Successful applicants must register as a Domiciliary Care Worker with Social Care Wales. (Registration and Inspection of Social Care (Wales) Act 2016.)

You must either have QCF qualification Level 2 in Care or be willing to work towards the qualification within a specified period of time, hold a full valid driving licence and have access to a vehicle insured for Class 1 business use. It is essential that you can travel extensively within the NPT authority.
