The role of the Peripatetic Social Worker is to provide a social work service within adult service in Neath Port Talbot County Borough Council. This is an exciting opportunity to develop a wide range of experience within adult services. You will have the opportunity to work with adults who have a range of needs, their carers and families.

As a Peripatetic Social Worker candidates will move between adult social work teams. The post holders will provide a high quality service and will work to support teams as required by the department.

Initially the peripatetic post will be based in The Afan Community Network Team, assessing individuals requiring a response to complex health and social care needs that have a substantial impact on them being able to achieve their identified personal well-being outcomes.

The successful applicants will be well organised, IT literate and be able to work under pressure and liaise with other professionals in line with statutory duties, policies and procedures.

The post holder will need a knowledge and working practice of the Well-being Act to include outcome focussed assessment and care planning. Knowledge and working practice of Continuing Health Care, Mental Capacity Act, and DoLS to include community welfare orders. Applications from newly qualified social workers welcome.
