Social Worker - 14 Plus Leaving Care and Care Experienced Children Team

We are currently looking for a Social Worker for our 14 plus team, which supports care experienced children and care leavers.

This post involves direct work with young people between the ages of 14 and 25 years and their carers and families in line with statutory LAC and Leaving Care requirements as outlined in the Social Services and Well-being Act 2014.

You will work with a number of statutory and voluntary agencies to co-ordinate the effective delivery of support services for young people in and leaving Local Authority care, in line with agreed plans and current legislation.

You will be responsible for assessing the needs of young people and facilitating and reviewing the delivery of LAC and Pathway Plans. As such, a high level of written and verbal English is required, alongside the ability to engage with complex young people and multi-disciplinary professional networks. Welsh language skills are desirable but not necessary.

The post requires a Social Work qualification. (DIPSW, Degree or Masters)
